On or about May 23, 2017, lamination, assembly, prep and chop employees were required to wear NIOSH approved full-face and half-face respirators during the manufacturer of fiberglass tanks and the employer did not establish and implement a written respiratory protection program that addressed the worksite-specific procedures such as but not limited to employee medical evaluations, fit testing, and training on the use, maintenance and care of the respirator. |
