Citation text line 29 CFR 1926.1053(a)(1)(i): Self supporting portable ladders were not capable of supporting, without failure, at least four times the maximum intended load: On or about May 25, 2016, at the above addressed jobsite, three employees were using self-supporting (A-Frame) ladder, to gain access to a ceiling mounted electrical junction box, for which the ladder was not capable of supporting: at least four times the maximum intended load, therefore resulting in the failure/collapse of the ladder and subsequent fall/serious injury of one of the three employees, and also resulting in exposing the other two employees to the same fall hazard. In the alternative, 29 CFR 1926.1053(b)(16): Portable ladders with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, corroded components, or other faulty or defective components, were not either immediately marked in a manner that readily identifies them as defective, or tagged with "Do Not Use" or similar language, and were not withdrawn from service until repaired: In the alternative, on or about May 25, 2016, at the above addressed jobsite, three employees were using a damaged self-supporting (A-Frame) ladder or not self-supporting (Extension) ladder, to gain access to a ceiling mounted electrical junction box, which resulted in the failure/collapse of the ladder and subsequent fall/serious injury of one of the three employees, and also resulting in exposing the other two employees to the same fall hazard.
