Employees installing roof shingles on a 40' x 40' foot wide roof at elevations of 6 feet or more above ground are not being protected from fall hazards by a fall protection system or an alternative fall protection measure under another provision such as the use of a monitoring system. The employer fails to assure effective protection against falls as required by the standard, as demonstrated on December 7, 2016, when employees installing roof shingles on a 3:12 pitch roof and flat roof at 5 Ocean Shore Drive, Ormond Beach, FL 32176, were exposed up to an 12.5 feet fall hazard above the ground in that they did not have an adequate safety monitor system nor fall protection system installed.
