On or about April 8, 2013, four employees removing and replacing shingles on the 5:12 pitch roof of Apartment Building A (Apartments 049-056) were not protected from a 15-foot fall hazard by a guardrail sytem, safety system, or personal fall arrest system. |

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend to at least 3 feet (.9m) above the upper landing surface to which the ladder was used to gain access nor was the ladder secured at its top to a ridged support and a grasping device, such as a grabrail, provided to assist the employee in mounting and dismounting the ladder: a. On or about April 8, 2013, at Apartment Building A, employees used a portable extension ladder which did not extend at least 3-foot above the landing surface, exposing themselves to a 15-foot fall hazard. |
