On or about February 8, 2015, the meat market manager and an employee worked in the vicinity of the Hobart Company meat (beef) cutting bandsaw that was left on and with the blade guide/guard raised up with approximately 6 inches of blade exposed, being exposed to an amputation hazard. The meat market manager's right forearm came in contact with the blade at the pint of operation and resulted in amputation of right forearm. |

Citation text line | 29 CFR 1910.1200(h)(3)(iv): The details of the hazard communication program developed by the employer, did not include an explanation of the labels received on shipped containers and the workplace labeling system used by their employer; the safety data sheet, including the order of information and how employee could obtain and use the appropriate hazard information: a. On or about February 9, 2015, employees in the meat market, deli and bakery departments worked with cleaning and sanitizing chemicals such as, cutting board whitener, score all purpose cleaner, sentry and shamrock dish detergents to clean and sanitize the machines, the employer had not explained the new labeling system and safety data sheets required under the new hazard communication program (GHS 2012). |
