(c) On or about April 3, 2015, at the above addressed worksite, the ChemTech PulsaFeeder pump on top of the BioExtend tote in the biodiesel storage area was wired with temporary wiring, run in series with another flow switch and then plugged into a wall receptacle, exposing employees to the hazards of electrical shock/burns and/or fire and explosions.
