On or about March 4, 2015, on the southeast side of a new commercial construction site, employees were using a staircase that had protruding conduit to which an employee could trip and fall, exposing employees to a fall and impalement hazards. |

On or about March 4, 2015, on the south side of a new commercial construction building, employees were using a breezeway with a wall opening that was not protected with guardrails, the employee was not protected from a 17 feet 2 inch fall hazard. b. On or about March 4, 2015, on the south side of a new commercial construction building, employees walking/working near an elevator shaft was not protected with guardrails, the employee was not protected from a 20 feet fall hazard. c. On or about March 4, 2015, on the southeast side of a new commercial construction building, employees walking/working around wall openings was not protected with guardrails, the employee was not protected from a 16 feet 4 inch fall hazard. |
