29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. a) For the jobsite at 4610 Eisenhower Blvd., Tampa, FL 33634, as occurring on or about 07/14/2016, employees accessed an upper level overhang for the entranceway in the front of the building to tape windows and paint. Employees were exposed to fall hazards of 16 ft. 6 in. off of the exposed edge of the overhang. No fall protection equipment was being used. |

29 CFR 1926.1060(a): The employer did not provide a training program for each employee using ladders and stairways, as necessary, which would enable each employee to recognize hazards related to ladders and stairways and train each employee in the procedures to be followed to minimize these hazards: a) For the jobsite at 4610 Eisenhower Blvd., Tampa, FL, as occurring on or about 07/18/2016 and weeks prior, employees had not been trained on ladder safety. On 07/18/2016 an employee was carrying a 24 foot ladder up another 24 foot ladder when the ladder slipped and he fell off of the ladder sustaining injuries and was hospitalized. A ladder safety training program was not provided to employees on the jobsite, and employees were exposed to ladder hazards from unsafe ladder operation. Ladder safety hazards included, but were not limited to, overreaching to access the area being painted and using ladders that were not at a safe angle. |
