29 CFR 1910.95(c)(1): The employer did not administer a continuing, effective hearing conservation program as described in 29 CFR 1910.9(c) through (o) whenever employee noise exposures equal or exceed an 8-hour time-weighted average sound level of 85 decibels measured on the A scale, or equivalently a dose of fifty percent: a) At 6300 New Tampa Highway, Lakeland, FL 33815, as observed on or about 11/16/2021: 1) For employees at the south end of site while building and repairing pallets exposed to a continuous noise level at 136.9 percent of the allowable 8 hour TWA (85 dBA). The equivalent dBA level for these dose percentage is 92.3 dBA. 2) For employees at the west side of site while building and repairing pallets exposed to a continuous noise level at 286.6 percent of the allowable 8 hour TWA (85 dBA). The equivalent dBA level for these dose percentage is 97.5 dBA. 3) For employees at the cut-down area while operating a trim saw exposed to a continuous noise level at 184 percent of the allowable 8 hour TWA (85 dBA). The equivalent dBA level for these dose percentage is 94.4 dBA. 4) For employees at the cut-down area while operating a chop saw exposed to a continuous noise level at 301 percent of the allowable 8 hour TWA (85 dBA). The equivalent dBA level for these dose percentage is 97.9 dBA. 5) For employees at the cut-down area while operating a band saw exposed to a continuous noise level at 534 percent of the allowable 8 hour TWA (85 dBA). The equivalent dBA level for these dose percentage is 102.1 dBA. |
