On July 12, 2016, on the 1:12 pitch roof, employees removing and installing metal deck roof panels were not protected from interior openings by the use of an adequate fall protection system, exposing employees to a 16 to 20 feet fall hazard. Jax Enterprises was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(10) which was contained in OSHA inspection number 1122871, citation number 1, item number 2 and was affirmed as a final order on March 25, 2016, with respect to a workplace located at 6940 Stuart Avenue, Jacksonville, Florida 32254. |
