On or about October 1, 2014, two employees installing a fascia on a 5:12 pitch roof framing were exposed to a 19 ft. 6 in. fall hazard without the use of personal fall arrest system, safety net system or guardrail system. b. On or about October 1, 2014, five employees engaged in bracing work on a 5:12 pitch roof framing were exposed to a 17 ft. fall hazard, and up to 21 ft. 6 in., without the use of personal fall arrest system, safety net system of guardrail system. |
