At 619 Ponte Vedra Boulevard, Ponte Vedra Beach, Florida: On or about July 26, 2022, the employer exposed an employee to fall hazards ranging from 6 to 41-feet, in that fall protection, such as, a body belt with a lanyard attached to the boom or basket was not utilized, while operating an aerial lift JLG, model 450 AJ, to cut and install siding on the front of the the newly constructed commercial building. b. At 619 Ponte Vedra Boulevard, Ponte Vedra Beach, Florida: On or about July 26, 2022, the employer exposed employees to fall hazards ranging from 6 to 41-feet, in that fall protection, such as, a body belt with a lanyard attached to the boom or basket was not utilized, while operating an aerial lift SkyJack, model SJ63AJ to cut and install siding on the front of the newly constructed commercial building. |
