Citation text line | 29 CFR 1918.62(g)(2)(iii): Synthetic web slings were not removed from service when they exhibited snags, punctures, tears, or cuts. a) For the synthetic web slings being used on or about 10/23/2012 to discharge the vessel Honesty Ocean. Slings observed in cargo hold #1 had visibly damaged threading, snags and damaged stitching. The slings were not padded in a manner to prevent cuts by the load, and they were being pulled from beneath the loads increasing deterioration by abrasive/mechanical action. |

Citation text line | 29 CFR 1918.81(a): Drafts were not safely slung before being hoisted. a) For the bundles of 42-foot, 3.5-inch diameter black pipe being hoisted from the vessel Honesty Ocean at Berth 201 at Port Tampa, on or about 10/23/2012. In that loads were not centered within the slings when being drafted. This practice is even more critical when using a 4-foot spreader bar for 42-foot pipe. Drafting an off-center load would cause the load to be off-balance which could result in the lower slings being pulled upward. This action would either just dump the load of pipe out of the end of the basket hitch, or cause the metal bands on the bundles to cut the slings. b) While loads were being moved, unused, extra synthetic web slings were allowed to hang freely from the loads to facilitate delivery of the extra slings. The freely hanging slings exposed the loads to be snagged on equipment and dumping the load. |
