On the NS1 transfer station platform and conveyor catwalk located at 1915 Wigmore Street, Jacksonville, Florida: On or about May 13, 2024, the employer exposed night shift supervisor and 2-other employees to fall hazards, in that the walking working surface with an unprotected side that is 4 feet or more above a lower level did not have a complete guardrail system and no other fall protection systems had been utilized. |

At the jetty chute conveyor drive motor, located at 1915 Wigmore Street, Jacksonville, Florida: On or about May 13, 2024, the employer exposed employees to struck-by material hazards, in that the conveyor drive motor had not been locked and tagged out by the authorized users while cleaning out the jetty chute below the jetty conveyor. |
