Warehouse - employees required to use fall protection equipment such as, but not limited to, harnesses and lanyards were not trained on the proper use and maintenance of equipment. A damaged shock absorbing lanyard was used for approximately 4 years without being replaced. The lanyard was already fully extended to 9'-7". In addition, another yellow strap which was used as a fall protection lanyard was extended to 11'-2". Violation observed on or about 3/7/13. |
