Citation text line OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that, employees operating Crown high reach powered industrial trucks (order pickers) were exposed to fall hazards of up to 20 feet onto a concrete surface. For the Famous Tate warehouse facility at 6405 N 54th St, Tampa, FL 33610, as observed on or about 07/22/2015, the following fall hazards were observed: a) Employees used Crown powered industrial trucks with elevating work platforms to access warehouse racking at heights of up to 20 ft. It was common practice for employees and management not to wear any type of fall restraint or fall arrest device while accessing upper levels, and guardrails were not available on all open/exposed sides of operating and working platforms.
