On or about July 16, 2014, during the night shift, employees were allowed to work in the feed area of the Koch wire drawing machine with illumination levels measured at 3 foot-candles (fc), exposing employees to struck-by and trip and fall hazards. Among other methods, a feasible abatement method to correct this hazard is to provide lighting that would increase illumination in accordance with the ANSI/IESNA RP-7-01: "Recommended Practice for Lighting Industrial Facilities".
