On or about October 22, 2020, employees were working near/within an opened/unguarded energized electrical cabinet/vault, that which contained live/120 Volt bus bars and related components/wiring, during which the employee was not wearing the required personal protective equipment (PPE) and clothing, nor was the electrical circuit de-energized and grounded or effectively guarded by insulation or other means; therefore exposing the employees to an electrical shock/arc flash hazard. |
