The single tieback employees used for the parapet clamp was not installed perpendicular to the face of the building. Further the tieback was improperly attached to the front of the parapet, which created an unsound anchorage point for the tieback. As a result, employees were exposed to a fall hazard. |

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: On or about January 18, 2017, at the above addressed jobsite, employees accessing the roof of a stairwell tower from an aerial lift; wherein the stair tower roof contained only a 18 inch parapet wall, and wherein the employees were dismounting the aerial lift, and then disconnecting their lanyards from the aerial life anchor, and then in-turn re-connecting their lanyards to an non-complaint anchor point (thought the parapet wall scupper drain), therefore exposing the employees to a fall hazard.
