Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to potential catastrophic releases of anhydrous ammonia, fire hazards, and high pressure explosion hazards: For the All-Temp Storage facility at 625 W. Bridgers Ave., Auburndale, FL 33823, as observed on or about 10/31/2013, employees were exposed to potential catastrophic releases of anhydrous ammonia, fire hazards, and high pressure explosion hazards due to the following conditions: a) Pressurized ammonia liquid and vapor lines in engine rooms #1,#3, and #4 were significantly corroded and in need of replacement. Paint was flaking off revealing significant corrosion underneath. b) High pressure receiver located outdoors of engine room #1 had paint flaking off and was rusting/corroding underneath. c) Pressure vessels in engine room #3 had damaged insulation and moisture and icing accumulation/intrusion underneath the damaged insulation. A vapor seal for insulated ammonia lines and pressure vessels was not maintained. d) Significant icing and moisture accumulation was present on ammonia lines and vessels in engine rooms #1, #3, and #4. e) Engine room #4 was located outdoors, exposing its compressors, pressure vessels, and piping to weathering resulting in significant insulation damage and corrosion. f) Many ammonia high pressure and low pressure lines throughout the facility were not marked to indicate direction of flow or physical state of ammonia inside piping lines. g) Compressors were inadequately labeled, in that, manufacturer model and serial numbers, year of manufacturer, hydrostatic test pressure, maximum allowable working pressure, refrigerant type, and rotation speed (RPMs) and information were not clear and able to be read from the compressor nameplates. h) In engine room #3, insulation was damaged on ammonia lines or missing altogether. Ammonia lines and equipment had severe icing and were corroded. i) Working ammonia detectors/sensors that were interlocked to alarms and interlocked to shut off unclassified electrical equipment were not present in engine rooms #1, #3, and #4. j) In engine room #1, an alarm for the failure of the ventilation fan was not present, and in engine room #3, no ventilation was present to remove air through and out of the engine room. Electrical equipment in all engine rooms was not rated for hazardous ammonia atmospheres. k) Pressure relief valves for all equipment associated with engine room #1 were not inspected and refurbished/replaced at the service intervals depicted on their nametags, and multiple pressure relief valves throughout the facility were missing their service nametags. l) Pressure relief valve discharge piping for engine room/system #1 was relieved to atmosphere only 5-10 ft. horizontally from a wall opening for engine room #1 (which was a ventilation intake for the engine room). The vent discharge was pointed downward into an isolated area beside engine room #1's exterior wall, containing an air intake opening for vapors to re-enter the engine room in event of an ammonia release. m) Pressure relief valve discharge piping for engine room/system #4 was relieved to atmosphere directly adjacent to the engine room at a height of only 2 feet above the engine room overhang roof (engine room #4 was not located inside a building, but rather outside with no walls). The overhang roof was only approximately 10 feet in height at the relief vent location. In event of a discharge, employees in the area of engine room #4 would likely be exposed to an ammonia cloud and potential IDLH ammonia concentrations. n) A preventative maintenance/inspection program was not in place to include regular inspection intervals for all equipment followed by replacement of damaged, defective, or due for service components. Components such as, but not limited to, pressure relief valves and ammonia pressure relief venting lines were out of service and inadequately designed/located. Piping and pressure vessels such as, but not limited to, ammonia accumulators, intercoolers, receivers, valves, and piping were corroded, had damaged insulation, and were in need of repair or replacement. Ammonia leaks were also present in coolers due to damaged/leaking strainers. |

Bridgers Ave., Auburndale, FL 33823, as observed on or about 10/31/2013, employees were exposed to potential immediately dangerous to life and health (ILDH) atmospheres due to anhydrous ammonia leaks while draining oil from oil pots in engine rooms and from ammonia leak sources from failed equipment and piping such as, but not limited to, leaking strainers and other sources which occurred on a continual basis. Respirators such as NIOSH certified full face piece pressure demand SCBA with a minimum service life of thirty minutes were not available. The only respirators available at the facility were N-95 filtering face piece respirators for particulates on a voluntary basis. |

Bridgers Ave., Auburndale, FL 33823, as observed on or about 10/31/2013, employees worked around ammonia leak sources at oil drains on a continual basis and around ammonia leak sources from failed equipment and piping such as, but not limited to, leaking strainers and other equipment as needed. Employees were not trained on specific work practice and emergency procedures for anhydrous ammonia leaks, nor were employees trained on specific procedures for personal protective equipment to be worn. Suitable respirators for potential immediately dangerous to life and health (ILDH) atmospheres were not available to employees performing oil drains, working in engine rooms, or for maintenance and response to ammonia leaks. |
