The procedure being used was not in accordance with the procedure specified in the machine Operator's Manual, on or about 05/09/13. b. The soil was previously disturbed type "C" soil. A protective system (shoring, shielding or sloping) was not provided for employees. Observed on or about November 7, 2012.
