On or about April 27, 2018, on the south side of the property, an employee operated and worked from an articulating boom aerial lift without the use of personal fall protection, being exposed to an approximate 12-foot fall hazard. Big Ben's Tree Service, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.67(c)(2)(v), which was contained in OSHA inspection number 951804, citation number 01, item number 001 and was affirmed as a final order on April 1, 2014, with respect to a workplace located at 13375 Beach Blvd, Jacksonville, Florida 32246. |
