On or about June 22, 2015, at the above addressed jobsite, employees were operating a wood jointer machine to cut 1/8 inch wide pieces of plexi-glass/plastic, for which the manufacturers point of operation guard/fence was not adequately adjusted (too wide) to accommodate the width of the material being cut, which resulted in the amputation of the tip of an employees left ring finger. NOTE: CERTIFICATION AND DOCUMENTATION OF ABATEMENT IS REQUIRED FOR THIS VIOLATION.
