On or about March 26, 2014, employees, at the Harrison Ave & 6th Street job site, were repairing a bell tower while working from an 80-foot tubular welded frame scaffold system. The scaffolding contractor completed assembly of the scaffold system on March 6, 2014, and the employer did not perform daily inspections of the scaffold and scaffold components by a competent person before each work shift. |

On or about March 25, 2014, employees, working on the 80-foot tubular welded frame scaffolding system, were moving an aluminum 24-inch wide by 20 foot long pick board to a new level when the pick board came into contact with 7,200 kV power lines, exposing the employees to electrocution and severe burns. |
