29 CFR 1926.95(a): Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, were not provided: On or about November 19, 2019, at the jobsite located at 2501 SW 145th Avenue, Miramar, FL 33027, the employer did not ensure an employee while performing roofing asphalt and adhesive application was provided with hand protection such as, chemical resistant gloves, while working with EverGuard TPO Bonding Adhesive, a roofing material, being used to replace the roof of the facility including, but not limited to, EverGuard TPO contains Toluene, Acetone Naptha, Phenolic Resin, Ethylbenzene (is classified as a 2B carcinogen-possibly carcinogenic to humans), Chlorlobenzene, Everguard TPO has a NFPA Health rating of 2 and a HMIS Hazard rating of 2. |

29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met: (Construction Reference: 1926.59): On or about November 19, 2019, at the jobsite located at 2501 SW 145th Avenue, Miramar, FL 33027, the employer did not develop, implement and/or maintain in the work location a written hazard communication program for employees performing roofing asphalt and adhesive application, and who were required to work with hazardous chemicals, such as, but not limited to, Asphalt, EverGuard TPO Bonding Adhesive, a roofing material, being used to replace the roof of the facility including, but not limited to, EverGuard TPO contains Toluene, Acetone Naptha, Phenolic Resin, Ethylbenzene (is classified as a 2B carcinogen-possibly carcinogenic to humans), Chlorlobenzene, Everguard TPO has a NFPA Health rating of 2 and a HMIS Hazard rating of 2. |
