Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc. For Meadow Burke establishment at 511 Santa Fe Road, Tampa, FL 33619, as observed on or about 10/15/2015, employees were exposed to amputation hazards as a result of operation of the following machinery: a) A Meadow Burke Beam Bolster was being operated without rotating parts being guarded, employee suffered right finger tip amputation when trying to clear a jam on or about 10/06/2015. |
