On or about, 4/20/2021 in the big warehouse # 1 , the employer failed to bolt down storage racks stacked with fishing merchandise to the floor, exposing the employees to struck-by and caught-in hazards when stacking and retrieving boxes and products. b.) On or about, 4/20/2021 in the small warehouse # 2 , the employer failed to bolt down storage racks stacked with boxed merchandise to the floor, exposing the employees to struck-by and caught-in hazards when stacking and retrieving boxes and products. c.)
