On or about June 5, 2018 one employee was working on a 24 inch wide plywood platform erected approximately 10 feet above the river bottom, to serve as a working platform to set concrete piles into a pile template for future pile driving into the river bed, and was not protected by any means of personal fall protection, guardrails or safety nets, exposing him to a fall of about 4 feet 8 inches, at high tide and 8 feet 2 inches at low tide into the Tolomato River below. High tide was estimated at 7:04 AM on June 5, 2018 |

Saboungi Construction, was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.106, which was contained in OSHA inspection number 934323, citation number 1, item number 2 and was affirmed as a final order on September 12, 2013, with respect to a workplace located at 2455 Boat Ramp Road, New Smyrna Beach, FL 32168. |
