Citation text line 29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) On or about January 13, 2020, at the machine shop, the DAKE metal cutting horizontal band saw - Model SE912 was missing the motor pulley cover, exposing employees to struck-by hazards created by rotating parts and ingoing nip points. b) On or about January 13, 2020, at the machine shop, the enclosure of the HAAS tool room mill (CNC)  Model TM1 was inadequate, in that, the enclosure design allowed access to the point of operation and other rotating parts from the top and sides, while it was not equipped with safeguarding devices such as presence sensing device. In addition, there was no hazard awareness devices around the table, exposing employees to struck-by and crush-by hazards created by the point of operation, moving table and rotating parts. c) On or about January 13, 2020, at the machine shop, the enclosure of the Mori-Seiki FM2 Vertical Machining Center was not equipped with safeguarding devices such as presence sensing device, exposing employees to stuck-by hazards created by the point of operation and rotating parts. d) On or about January 13, 2020, at the machine shop, the enclosure of the Tsugami BE20-V was not equipped with safeguarding devices such as presence sensing device, exposing employees to stuck-by hazards created by the point of operation and rotating parts. e) On or about January 13, 2020, at the machine shop, the enclosure of the Mori Seiki SL-2HS  2416 (MV Junior X/Y/Z axis) was inadequate, in that, the enclosure design allowed access to the point of operation and other rotating parts from the top and sides, while it was not equipped with safeguarding devices such as presence sensing device. In addition there was no awareness device around the table, exposing employees to struck-by and crush-by hazards created by the point of operation, moving table and rotating parts. f) On or about January 13, 2020, at the machine shop, the enclosure of the FADAL VMC-4020 CNC-88HS was not equipped with safeguarding devices such as presence sensing device, exposing employees to stuck-by hazards created by the point of operation and rotating parts. g) On or about January 13, 2020, at the machine shop, the enclosure of the MORI SEIKI NL 1500 CNC Lathe was not equipped with safeguarding devices such as presence sensing device, exposing employees to stuck-by hazards created by the point of operation and rotating parts. h) On or about January 13, 2020, at the machine shop, the enclosure of the MORI SEIKI CL-150 CNC Lathe was not equipped with safeguarding devices such as presence sensing device, exposing employees to stuck-by hazards created by the point of operation and rotating parts.
