On June 5, 2019, on the 6:12 pitch roof, two employees installing metal framing and brass panels were not adequately protected from a fall hazard that ranged from 9 feet to 80 feet in that they did not attach their harnesses to the rope and anchor system. b. On June 5, 2019, on the lower flat roof, employees approached an unprotected edge to access material storage, exposing themselves to a 50 feet fall hazard.
