I've contacted Simon Agency Inc regarding the report and they admit that the name is incorrect. However, they are still refusing to remove this false information from my credit. I informed them that I was a victim of identy theft and they are refusing to comply.
Attached is a copy of the FTC report updating adding them to my fraudulent accounts.

I am writing to follow up on my second dispute regarding the accuracy and validity of a collection account reported on my consumer credit report. Despite the collection agency 's commitment to provide supporting documentation, I have yet to receive the requested validation. This continued failure to comply with regulatory requirements necessitates further investigation. The debt has now been transferred to a different collection agency. Rather than providing the requested information, they issued a vague statement and transferred the debt.

* Account Number : * Amount : {$2900.00} Laws Being Violated : 1. XXXX Violations : The collection agency has accessed and disclosed my individually identifiable health information without proper authorization, in violation of the Health Insurance Portability and Accountability Act of XXXX ( XXXX ). The specific violations include : * Unauthorized use of my unique health identifier.
* Obtaining my individually identifiable health information without my consent.
* Disclosing my health information to unauthorized parties.
These actions are punishable under XXXX, with penalties including fines and imprisonment as detailed in Public Law 104-191.
2.FCRA Violations : The collection agency is also in violation of the Fair Credit Reporting Act ( FCRA ), which ensures the accuracy, fairness, and privacy of information in consumer credit reports. The specific violations include : * Failure to provide validation information as required under 12 CFR 1006.34 ( b ) ( 5 ), despite having placed a collection account on my credit report.
* Not providing me an opportunity to dispute the account according to 12 CFR 1006.34 ( c ) ( 4 ) ( i ).
* Utilizing false, misleading, and deceptive means to collect the debt, thereby violating my consumer rights under the FCRA.

where did the debt originate? I sent a letter directly to Simon 's Agency requesting this information via US mail over 30 days ago and have yet to receive a response.
I respectfully request that you provide me with the following : 1. the amount of the debt ; 2. the name of the creditor to whom the debt is owed ; 3. proof of the last payment made on the account if any.
