Citation text line 29 CFR 1910.178(p)(1): Powered industrial truck(s) found to be in need of repair, defective, or in any way unsafe had not been taken out of service until restored to safe operating condition(s): a. On or about 04/04/2023, in the warehouse area of an establishment located at 940 West 13th Street in Riviera Beach, Florida, employees were exposed to fall and caught-between hazards when operating a Toyota Forklift, model #5FGC25, serial #11952 that did not have seat belt where there was a bracket provided by the manufacturer for the placement of a seat belt, was found to be in need of repair and was not removed from service until restored to a safe operating condition. b. On or about 04/04/2023, in the warehouse area of an establishment located at 940 West 13th Street in Riviera Beach, Florida, employees were exposed to fall and caught-between hazards when operating a Toyota Forklift, model #42 6F GCU25, serial #71351 that did not have seat belt where there was evidence that a seatbelt had previously been in place, was found to be in need of repair and was not removed from service until restored to a safe operating condition.
