At the site - employees exposed to fall hazards were not provided with training to recognize hazards associated with working on a scaffold. Employees did not know at what height fall protection was required while on a scaffold. Employees did not know what was needed for a guardrail system and did not know that a cross brace could only serve as a top or mid-rail, but not both. Employees did not know at what height the top and mid-rails needed to be installed. Violation observed on or about 2/11/14.
