On or about May 13, 2022, at the facility, employees were changing-out rotary die cutting boards on a rotary die cutter machine, during which time the energy control procedure (Intermediate Energy State/ IES procedure) did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy; therefore, exposing the employees to amputation hazards. b. On or about May 13, 2022, at the facility, employees were conducting weekly Preventative Maintenance/ PM, such as weekly cleaning of the rotary die cutter machine, during which time the energy control procedure (Zero Energy State/ ZES procedure) did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy; therefore, exposing the employees to amputation hazards.
