Citation text line | 29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toeboards: a. On or about 08/14/2014, at the launch pad - employees were exposed to a 12'- 7" fall hazard while working on the launch pad, in that, fall protection was not used. The outer perimeter guard rails were removed and were not re-installed prior to the start of work. An employee fell approximately 9 feet onto hydraulic hoses after the throwback struck the employee and knocked him off the launch pad. b. On or about 08/14/2014, at the launch pad - employees were exposed to an approximate 60 foot fall hazard while working on the launch pad near the flame pit, in that, fall protection was not used. The work platform deck plates for the flame pit were removed and were not re-installed prior to the start of work. Fall protection was not used. |
