On or about September 13, 2016, the employer did not conduct a hazard assessment, which would have identified that hazards of working near a 3-phase overhead power line system. The overhead power line system was not protected, in that the utilities company was not contacted for de-energization, the power lines were not sleeved, and no dielectric personal protective equipment was being worn, which resulted in an employee being electrically shocked.
