In the welding department's gouging booth, on or about August 8, 2016, the employer did not ensure that an electrical transformer was cleaned so thoroughly as to make absolutely certain there were no flammable materials present, which directly resulted in an explosion and the fatal injury of the employee who was allowed to conduct reverse welding (gouging) operations on the unclean container.
