In locations where flammable vapors were present, the employer did not take precautions to prevent ignition by eliminating or controlling chemical and physical-chemical reactions. a. Center of Plant Tank Farm (Area 22): On or about September 13, 2022, fourteen employees were exposed to struck-by/crushing hazards, in that stored residue materials contained in a 12,500lbs capacity tank generated an exothermic reaction that propelled the tank in the air, due to over pressure, landing approximately 100ft east of its position in the tank farm. The employer did not take precautions to prevent ignition by eliminating or controlling chemical and physical-chemical reactions. |
