On or about February 1, 2023, at the pre-fabrication shop; the employer did not ensure that employees, cutting notches from wood trusses with a handheld DeWalt 7-1/4" Worm Drive Circular Saw (Model: DWS535B (120V and 4800/min (RPM)), secured the work piece to a stable surface in order to allow a firm grip with both hands on the saw and away from the cutting area.

Dual Bevel Miter Saw, Model: R4123 (120V and 4000/min (RPM)) and S/N: GW19272D2A20584, did not cover the blade entirely when the tool was withdrawn from the stock, exposing operators to struck-by hazards created by turning blade causing lacerations, cuts and / or, amputations.
