29 CFR 1910.212(a)(1):One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a. On or about January 4, 2021, at the Bindery Department, the employer allowed the Zenith #A three knife trimmer machine to be operated with the rear guard door open, in that the door pin was broken and interlock safety switch was disconnected, which allowed the operator to run the machine with the door open, exposing employees to amputation hazards. b. On or about January 4, 2021, at the Bindery Department, the employer allowed the Zenith #B three knife trimmer machine to be operated without the protective guard at the rotating motor belts, exposing employees to caught-in hazards. c. On or about January 4, 2021, at the Bravo #2 in the Bindery Department, the employer allowed the RIMA stackers to be operated without protective guards in the feeder belt area and turntable/stack pusher area, exposing employees to caught-in and amputation hazards. d. On or about January 4, 2021, at the Bravo #1 and Norm A in the Bindery Department, the employer allowed the RIMA stackers to be operated without protective guards in the turntable/stack pusher area, exposing employees to caught-in and amputation hazards. The employer, Trend Offset Printing Services, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.212(a)(1), which was contained in OSHA inspection number 1356685, citation number 1, item number 1 and was affirmed as a final order on April 24, 2019, with respect to a workplace located at 10301 Busch Drive North, Jacksonville, FL 32218. |
