On or about January 4, 2021, at the Bindery Department, the employer allowed the Zenith #A three knife trimmer machine to be operated with the rear guard door open, in that the door pin was broken and interlock safety switch was disconnected, which allowed the operator to run the machine with the door open, exposing employees to amputation hazards. b. On or about January 4, 2021, at the Bindery Department, the employer allowed the Zenith #B three knife trimmer machine to be operated without the protective guard at the rotating motor belts, exposing employees to caught-in hazards. c. On or about January 4, 2021, at the Bravo #2 in the Bindery Department, the employer allowed the RIMA stackers to be operated without protective guards in the feeder belt area and turntable/stack pusher area, exposing employees to caught-in and amputation hazards. d. On or about January 4, 2021, at the Bravo #1 and Norm A in the Bindery Department, the employer allowed the RIMA stackers to be operated without protective guards in the turntable/stack pusher area, exposing employees to caught-in and amputation hazards.
