Results were calculated based on the 446 minute sampling period averaged with no exposure assumed for the 34 minute period not sampled. b) Concrete mix bagging area, employee #2; On July 2, 2024, and at times prior to the employer exposed employees to an 8 hour calculated Time Weighted Average (TWA) of 11 milligrams per cubic meter of air which is 2.2 times the calculated 8 hour time weight average Permissible Exposure Level (PEL) of 5 milligrams per cubic meter of air while bagging concrete mix. Riviera Beach, FL 33407 production facility, an employee fabricating pavers (OMAG dry Operator) was exposed to a respirable crystalline silica level of 74.092 micrograms/cubic meter, which exceeds the permissible exposure limit (PEL) of 50 micrograms/cubic meter. The sampling was performed for 474 minutes, and zero exposure was assumed for the unsampled time. b) On or about June 7, 2019, at 7167 Interpace Rd. Riviera Beach, FL 33407 production facility, an employee fabricating pavers (OMAG dry side operator) was exposed to a respirable crystalline silica level of 58.346 micrograms/cubic meter, which exceeds the permissible exposure limit (PEL) of 50 micrograms/cubic meter. The sampling was performed for 480 minutes. The Old Castle APG South Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.1000(c), which was contained in OSHA inspection number 1035453, citation number 1, item number 2a and was affirmed as a final order on 07/26/2016, with respect to a workplace located at 4083 Bonsal Rd Conley Georgia, 30288. |

29 CFR 1910.147(f)(1)(v): All systems were not de-energized and energy control measures reapplied in accordance with paragraph (d) of this section to continue the servicing and/or maintenance: a) At plant one located at 421 Leonard Boulevard West, Lehigh Acres, FL, an employee involved in cleaning inside of mixer # 1, Wiggert & Co HPGM 1500, was crushed to death by the mixer arm support / paddles, as observed on or about 4/13/2016. The mixer was not de-energized and lock device reapplied after the mixer had been jogged to move the paddles to the desired location so they could be cleaned from the outside through the open mixer door. |
