On or about April 18, 2013, Pre-Op and PACU Nurses, OR Nurses, and Surgical Technicians gave injections, started IVs or assited the surgeons in the operation rooms handling sharps on a daily basis and the employer failed to provide bloodborne pathogens training to new hired employees upon initial assignment as required under 29 CFR 1910.1030(g)(2)(ii)(A). b. On or about April 18, 2013, Pre-Op, PACU, and OR Nurses and Surgical Technicians gave injections, started IVs or assisted the surgeons in the operation room handling sharps on a daily basis and the employer failed to provided bloodborne pathogens training annually to existing employees as required under 1910.1030(g)(2)(ii)(B). |

Employers shall ensure that each employee has access to labels on containers of formaldehyde and to safety data sheets, and is trained in accordance with the requirements of HCS and paragraph (n) of this section: a. On or about April 18, 2013, at the pathology laboratory, employees poured into a container a 10% Formalin solution to preserve human tissue and the employer did not provide training on the health hazards associated to handling of formaldehyde and did not include formaldehyde in their hazard communication program or training program. |
