On or about 04/09/2013, at the warehouse - employees were exposed to a fall hazard, in that, they were required to move items to and from racks using A-frame ladders, and they were required to carry items while ascending and descending the A-frame ladders. b. On or about 05/28/2013, at the warehouse - employees were exposed to a fall hazard, in that, the casters were not locked on the mobile ladder stand platform and the ladders were moved while employees were still standing on the mobile ladder stand platforms. c. On or about 05/28/2013, at the warehouse - employees were exposed to fall hazard, in that, they were allowed to work from the mobile ladder stand platform that were not equipped with intermediate rails that were mid-way from the top rail or if chains were used as rails they were not latch or it was mid-way from the top guard rail to the top of the platform.
