THE LITERATURE IS REPLETE WITH OBSERVATIONAL STUDIES PURPORTING A ROLE OF THE GUT MICROBIOME IN BODY WEIGHT REGULATION. THE MAIN GAP IN THE DEVELOPMENT OF MICROBIOME-TARGETED THERAPEUTICS TO TREAT OBESITY IS A DEEP UNDERSTANDING OF THE CAUSAL MECHANISMS BY WHICH MICROBES IMPACT ENERGY BALANCE IN HUMANS. IN A FIRST OF ITS KIND STUDY, WE IMPLEMENTED A QUANTITATIVE BIOENERGETICS PARADIGM IN WHICH HUMAN ENERGY BALANCE WAS DEEPLY AND PRECISELY PHENOTYPED (ENERGY INTAKE, EXPENDITURE AND FECAL ENERGY LOSS) DURING AN INPATIENT RANDOMIZED CONTROLLED CROSSOVER FEEDING STUDY. WE USED DIETS THAT DELIVERED MINIMAL (WESTERN DIET; WD) VERSUS HIGH (MICROBIOME ENHANCER DIET; MBD) AMOUNTS OF MICROBIOTA FERMENTABLE DIETARY SUBSTRATES TO THE COLON TO REPROGRAM THE GUT MICROBIOME WITHIN INDIVIDUALS. WE UNCOVERED DIET-HOST-MICROBE INTERACTIONS THAT IMPACTED HUMAN ENERGY BALANCE VIA INCREASED FECAL ENERGY LOSS AND THUS, LOWER METABOLIZABLE ENERGY (ME) THAT WAS DUE IN PART TO AN INCREASE IN MICROBIAL BIOMASS ON THE MBD AS COMPARED TO THE WD. TO ADVANCE THIS WORK, WE NEED TO UNDERSTAND THE QUANTITATIVE AND FUNCTIONAL MECHANISMS BY WHICH THE GUT MICROBIOME IMPACTS ME WHEN EXPOSED TO DIFFERENT DIETS. WE PROPOSE TWO AIMS TO ACCOMPLISH THIS. IN AIM 1A, WE WILL DETERMINE THE AMOUNT OF FECAL ENERGY LOSS THAT IS DUE TO SHUNTING OF ENERGY FROM THE HUMAN DIET TOWARDS BIOMASS EXPANSION (AND THUS AWAY FROM THE HOST) TO TEST THE HYPOTHESIS THAT THE DIETARY ENERGY DIVERTED FROM HOST TOWARDS BIOMASS EXPANSION IS LARGE ENOUGH TO PROMOTE LOWER ME (NET NEGATIVE ENERGY BALANCE) ON THE MBD VS. THE WD. IN AIM 1B, WE WILL PERFORM ABSOLUTE QUANTIFICATION OF MICROBIAL SPECIES TO DETERMINE WHICH ONES ARE THE DOMINANT PLAYERS IN BIOMASS EXPANSION AND THEIR PROPORTIONAL CONTRIBUTION TO FECAL BIOMASS ENERGY. IN AIM 2A, WE WILL IMPLEMENT A NOVEL INTEGRATED MULTI-OMICS WORKFLOW AIMED AT DETERMINING MICROBIAL FUNCTIONS AND SUBSTRATE PREFERENCES IN VIVO. THESE DATA WILL TELL US WHAT THE MICROBES “EAT”, WHICH SPECIFIC SPECIES COMPETE FOR THE SAME DIETARY SUBSTRATES, AND THE ASSOCIATED FUNCTIONS OF THE MICROBIAL COMMUNITY WHEN SPECIFIC DIETARY SUBSTRATES ARE AVAILABLE. THESE DATA WILL BE VALIDATED IN AIM 2B BY PERFORMING EX VIVO EXPERIMENTS TO CHANGE THE ABSOLUTE ABUNDANCE OF SPECIFIC MICROBES THAT CONTRIBUTE MOST TO BIOMASS BY ADDITION OF THE PREFERRED SUBSTRATES OF THOSE MICROBES. EXERCISE, STRESS BIOMARKERS, AND ALZHEIMER'S DISEASE NEUROPATHOLOGY - ABSTRACT AEROBIC EXERCISE MIGHT AFFECT THE BRAIN THROUGH A PLURALITY OF PATHWAYS, INCLUDING VIA MODULATION OF THE HYPOTHALAMIC-PITUITARY-ADRENAL (HPA) AXIS. THE HPA AXIS REGULATES THE RELEASE OF CORTISOL, A GLUCOCORTICOID HORMONE THAT IS RELATED TO IMPAIRED COGNITIVE PERFORMANCE, REDUCED BRAIN VOLUME AND FUNCTION, AND INCREASED RISK FOR ALZHEIMER’S DISEASE (AD), INCLUDING MARKERS OF AD PATHOPHYSIOLOGY. MANY OF THE SAME BRAIN REGIONS AND COGNITIVE DOMAINS AFFECTED BY HIGH CORTISOL LEVELS ARE THE SAME THAT ARE POSITIVELY AFFECTED BY ENGAGING IN REGULAR AEROBIC EXERCISE. THUS, WE HYPOTHESIZE THAT AEROBIC EXERCISE-INDUCED CHANGES IN CORTISOL MIGHT ACT AS A PATHWAY BY WHICH LONG-TERM EXERCISE INFLUENCES NEUROCOGNITIVE HEALTH. FURTHER, WE SPECULATE THAT AEROBIC EXERCISE MIGHT INFLUENCE AD NEUROPATHOLOGY THROUGH ITS INFLUENCE ON CORTISOL. ADDRESSING THESE GAPS COULD HAVE FAR-REACHING APPLICATION FOR ADOPTING EXERCISE AS A METHOD OF PREVENTING OR TREATING COGNITIVE IMPAIRMENT. WE ADDRESS THESE UNANSWERED QUESTIONS IN THIS HIGH-RISK/HIGH-REWARD R21 APPLICATION. THE PROPOSED STUDY WILL CONDUCT AN ANALYSIS OF HAIR CORTISOL CONCENTRATION (HCC) IN A LARGE AND UNIQUELY WELL-CHARACTERIZED SAMPLE OF COGNITIVELY NORMAL OLDER ADULTS (N=648). HAIR SAMPLES HAVE BEEN COLLECTED AND BANKED BOTH BEFORE AND AFTER A MULTI-SITE PHASE III DOSE-RESPONSE 12-MONTH SUPERVISED RANDOMIZED CLINICAL TRIAL OF EXERCISE TITLED INVESTIGATING GAINS IN NEUROCOGNITION IN AN INTERVENTION TRIAL OF EXERCISE (IGNITE). THE IGNITE STUDY AND THE BANKED HAIR SAMPLES IN N=648 WILL SERVE AS THE BASIS FOR THE PROPOSED STUDY. THIS PROPOSAL REFLECTS AN UNPRECEDENTED OPPORTUNITY TO DETERMINE WHETHER AEROBIC EXERCISE-INDUCED CHANGES TO HCC STATISTICALLY MEDIATES CHANGES TO COGNITION, AD NEUROPATHOLOGY, BRAIN VOLUME OR FUNCTION, AND IS IN LINE WITH THE NIH MISSION TO LEVERAGE EXISTING COHORT AND RCT SAMPLES FOR COST-EFFECTIVELY TESTING NEW HYPOTHESES. OUR PRIMARY AIMS INCLUDE: AIM 1. EXAMINE WHETHER EXERCISE-INDUCED CHANGES IN HCC STATISTICALLY MEDIATES COGNITIVE CHANGES RESULTING FROM THE INTERVENTION. AIM 2. THE ROLE OF POOR WAT QUALITY IN IR, HOWEVER, IS LESS CLEAR. LOW ADIPOCYTE TRIGLYCERIDE (TG) TURNOVER, HIGH FORMATION RATES OF NEW ADIPOCYTES, HIGH SENESCENT CELL BURDEN, TISSUE AND CELLULAR INFLAMMATION AND HIGH CIRCULATING FREE FATTY ACIDS (FFAS) ALL CHARACTERIZE POOR WAT QUALITY. EXERCISE ENHANCES WHOLE-BODY IR AND DECREASES FASTING FFAS, INDEPENDENT OF REDUCTIONS IN FAT MASS, THUS HIGHLIGHTING THE SIGNIFICANCE OF EXERCISE EFFECTS ON WAT QUALITY. EVIDENCE OF THE DIRECT IMPACT OF EXERCISE – INDEPENDENT OF WEIGHT LOSS – ON IR WITHIN WAT IS LIMITED AND INCONCLUSIVE. THE UNDERLYING MECHANISMS THAT DRIVE EXERCISE-INDUCED CHANGES IN WAT BIOLOGY, SUCH AS NEW ADIPOCYTE FORMATION AND TG TURNOVER RATES, CELL COMPOSITION, SENESCENCE, AND INFLAMMATION, ARE POORLY UNDERSTOOD IN HUMANS. OURS WILL BE THE FIRST STUDY TO COMPREHENSIVELY ASSESS THE DIRECT EFFECTS OF EXERCISE ON WAT-SPECIFIC IR AND WAT QUALITY IN VIVO AND EX VIVO IN HUMANS. WE HAVE CONDUCTED GROUNDBREAKING STUDIES OF IN VIVO FORMATION OF NEW ADIPOCYTES AND ADIPOCYTE TG TURNOVER RATES USING A METABOLIC LABELING PROTOCOL THAT INCORPORATES DEUTERIUM (2H), ADMINISTERED AS 2H2O, INTO THE DNA AND TRIGLYCERIDES OF ADIPOSE CELLS. WE NOW HAVE COMPELLING PUBLISHED AND PRELIMINARY EVIDENCE THAT EXERCISE REDUCES IN VIVO FORMATION OF NEW ADIPOCYTES IN BOTH MICE AND HUMANS, SUGGESTING AN IMPORTANT EXERCISE-INDUCED MECHANISM OF WAT REMODELING. WE REASON THAT INCREASED ADIPOCYTE TG TURNOVER RATES IMPROVE ADIPOCYTE-SPECIFIC AND OVERALL WAT QUALITY AND THUS ABROGATE THE NEED FOR INCREASED FORMATION OF NEW ADIPOCYTES. USING OUR ESTABLISHED METHOD, WE WILL EMPLOY INNOVATIVE SINGLE NUCLEI TRANSCRIPTOMICS (SNRNASEQ) TO DISCOVER THE EFFECTS OF EXERCISE ON SENESCENCE AND INFLAMMATION WITHIN ADIPOCYTES AND NON-ADIPOCYTES OF WAT TO FURTHER UNDERSTAND HOW EXERCISE IMPACTS WAT QUALITY AND WAT- SPECIFIC IR VIA IN VIVO MICRODIALYSIS DURING INSULIN SUPPRESSION. IN THIS STUDY, WE WILL PERFORM A RANDOMIZED, CONTROLLED CLINICAL TRIAL TO EVALUATE THE IMPACT OF 12 WEEKS OF ENDURANCE EXERCISE ON ABDOMINAL SUBCUTANEOUS WAT ADIPOCYTE FORMATION AND TG TURNOVER IN VIVO, CELL COMPOSITION AND WAT-SPECIFIC LIPOLYTIC RATES UNDER PHYSIOLOGICAL INSULIN SUPPRESSION IN MEN AND PRE-MENOPAUSAL WOMEN WITH OBESITY. WE HYPOTHESIZE THAT ENDURANCE EXERCISE WILL INCREASE ADIPOCYTE TG TURNOVER RATES THEREBY DECREASING FORMATION OF NEW ADIPOCYTES, THE SIZE OF EXISTING ADIPOCYTES AND REDUCING INFLAMMATION AND SENESCENT CELL BURDEN IN ADIPOCYTES AND NON- ADIPOCYTES WITHIN WAT. PREVIOUS RESEARCH HAS IDENTIFIED A DYSREGULATED TRANSCRIPTIONAL PROFILE WITH SKM IR IN THE BASAL STATE AND DURING INSULIN STIMULATION FROM A HYPERINSULINEMIC EUGLYCEMIC CLAMP (HE). A SIGNIFICANT LIMITATION OF THESE PRIOR STUDIES IS THE RESTRICTION TO WHOLE SKM HOMOGENATES AND THEREFORE NOT IDENTIFYING WHICH CELLS OR SPATIAL AREA THE DYSREGULATED SIGNALS ORIGINATE FROM. WE HAVE DEVELOPED A PIPELINE FOR 3’ AND 5’ AMPLIFICATION OF SINGLE CELLS OR NUCLEI FROM SKM USING FULL-LENGTH SMART-SEQ TECHNOLOGY WITH THE ICELL8 PLATFORM, RESULTING IN 2-3 FOLD GREATER GENE COVERAGE THAN PREVIOUS RESEARCH, ALLOWING US TO INVESTIGATE TRANSCRIPTIONAL ABERRATIONS AT A SINGLE CELL/NUCLEI RESOLUTION. RECENT DEVELOPMENTS IN SPATIAL TRANSCRIPTOMICS NOW PERMIT TRANSCRIPTIONAL PROFILING ON SECTIONED TISSUE. IN THIS PROPOSED RESEARCH, WE WILL LEVERAGE BOTH FULL-LENGTH SINGLE CELL AND SINGLE NUCLEI RNA-SEQ AND SPATIAL TRANSCRIPTOMICS TO DETERMINE CELL- AND SPATIALLY-DISTINCT TRANSCRIPTIONAL ABERRATIONS IN SKM THAT OCCUR WITH IR IN T2D. THE CAREER DEVELOPMENT TRAINING WILL REFINE MY SKILLS IN; LEADING AN INDEPENDENT LAB, BIOINFORMATICS, AND HUMAN CLINICAL RESEARCH WITH A FOCUS ON ASSESSING INSULIN-STIMULATED GLUCOSE DISPOSAL (RD) AND NON- OXIDATIVE GLUCOSE DISPOSAL (NOGD) USING HYPERINSULINEMIC EUGLYCEMIC (HE) CLAMPS WITH GLUCOSE TRACERS AND INDIRECT CALORIMETRY. MY K99/R00 TRAINING WILL UNIQUELY POSITION ME TO CONDUCT BEDSIDE-TO-BENCH-TO-BIOINFORMATICS RESEARCH TO UNCOVER THE MOLECULAR PROFILES OF SKM IR. FOR THE FIRST TIME, THE TRANSCRIPTIONAL RESPONSES TO INSULIN IN HUMAN SKM WILL BE PROBED AT A SINGLE CELL/NUCLEI AND SPATIAL RESOLUTION. WE WILL IDENTIFY WHICH CELLS AND AREAS OF SKM RESPOND TO INSULIN AND IF THEY ARE SPATIALLY LOCALIZED TO EACH OTHER AND IF THEIR GENE-NETWORK PROFILES CORRELATE WITH A GREATER IN VIVO RD AND NOGD ASSESSED SIMULTANEOUSLY WITH A HE CLAMP. THIS NOVEL AND INNOVATIVE APPROACH WILL REVEAL WHICH CELLS (AND IMPORTANTLY, WHERE IN SKM) THE ABERRATIONS OCCUR WITH IR IN T2D. THE TRANSLATIONAL RESEARCH INSTITUTE AT ADVENTHEALTH IS AN IDEAL ENVIRONMENT FOR DEVELOPING MY BEDSIDE-TO- BENCH-TO-BIOINFORMATICS RESEARCH APPROACH WITH CUTTING-EDGE METABOLIC TRANSLATIONAL RESEARCH FACILITIES AND A RICH INTERDISCIPLINARY ENVIRONMENT.
