On or about July 1, 2014, above the dryer in front of the electric planner in the Wash Bay, the 6-outlet flexible surge protector power strip was not used in accordance with instructions included on the labeling and it had two temporary flexible cords plugged into it, to energize a small vacuum and a fan, a third cord plugged into it to energize the dryer, a fourth cord plugged into it for powering a microwave, and a fifth cord plugged into it for a radio, exposing employees to an electrical shock hazard. |

The employee was also required to wear 3M dust mask while performing sanding activities in the spray booth.
