On or about August 25th, 2016, at the jobsite located at 7590 W 19th Ct. Hialeah, FL 33014, the employer did not ensure that employees use safety glasses that could be worn over the prescription lenses without disturbing the proper position of the prescription lenses or the protective lenses to avoid flying metals part that maybe produced during the rivets manufacturing.
