On or about August 30, 2017, at the intersection of SR 80 and County Road 833, North Side, the employer had not provided any type of cave-in protection for two employees that had been sent inside the excavation which was 7'6" in depth, 22' in length, and 18'6" in width, and class C soil, exposing the employee to an engulfment hazard. |

29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury: a. At the site, employees were exposed to struck by hazards, in that, training and education was not provided relating to safety requirements for excavations, to include at a minimum, shoring and sloping requirements, the various types of soil classifications, placing and handling the spoil so that it did not present a hazard to employees, safe means of egress to and from an excavation, on or about 10/21/2015. b. At the site, employees were exposed to a struck-by hazard, in that, training and education was not provided relating to hazards such as, but not limited to, handling material while inside a trench by using the excavator bucket to lower material such as gravel over the employees, on or about 10/21/2015. |

On Edgewater Drive west of the intersection with Midway Blvd, employees were exposed to a struck by hazard while installing a 10 inch sanitary sewer pipe and a 12 inch reclaim water pipe, in that, excavators were driven on top of the spoil which was placed right at the edge of the north excavation wall which was 10 feet deep and 250 feet long, on or about 10/21/2015. |
