OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck by and crushing hazards during the operation of four 1-Ton HSI Model 512 Adjustable Height Gantry Cranes with 1-Ton Mini Electric Hoists: a) On or about January 10, 2020, at the maintenance shop, the swivel wheels of the1-Ton HSI Model 512 Adjustable Height Gantry Crane were not locked, chocked or secure when lifting a 1,336 pound lawn mower load allowing the crane to move, exposing employees to struck-by and crushing hazards. b) On or about January 10, 2020, at the maintenance shop, the bolts, nuts, beveled washers and plates connecting the I-beams to the vertical legs of the1-Ton HSI Model 512 Adjustable Height Gantry Cranes were not inspected to ensure proper installation and torque, exposing employees to struck-by and crushing hazards. c) On or about January 10, 2020, at the maintenance shop, the employer did not ensure that employees lifted equipment vertically without side pulls when using 1-Ton HSI Model 512 Adjustable Height Gantry Cranes, exposing employees to struck-by and crushing hazards. d) On or about January 10, 2020, at the maintenance shop, the wire ropes of the 1 Ton Mini Electric Hoists had deficiencies, such as but not limited to, kinks, twists, abrasions, bird caged wire, and hoist rope pulled out of the sheaves or across drum grooves. e) On or about January 10, 2020, at the north side of the service shop towards the front of the Manager Office, the hook of the gantry crane had a damaged latch that did not close effectively. |
